Final Physical Documents

Here is what you need to know

Final physicals go by different names depending on your branch of service. Final Physicals can sometimes be referred to as a Separation History & Physical Exam (SHPE) or a Separation Health Assessment (SHA). You can read more about the DoD requirement here. Do not go into this exam blind. There are VERY specific ways to approach the conversation with your PCM and how to establish service-connection. Zero Nexxus will assess your occupational and environmental exposures to ensure you have not overlooked claimable conditions and we will help you advocate for yourself.

With greater commitment to service and a higher optempo, health often gets sidelined. It’s not surprising when a service member separates from service to find they have no injuries or illnesses documented in their medical record. This shortcoming can create several problems when separating from service, most notably when submitting a VA Claim. Many veterans fall victim to a feedback loop with the VA that can extend years into their veteran lifecycle. The frustration is real when you must explain how your back pain is related to static line jumps or rucking 20 miles, yet still receive a ‘not service-connected’ rating for lower back pain.

Your final physical is one of the last opportunities to capture service-connected conditions while still on active duty. The forms required to complete this process can be downloaded below.
